	New Hire
	FEIN
	State UI Number (UI#)
	New Hire Contact Information
	Payroll/Child Support Withholding Address Information
		This is the address where a child support withholding order filed against the employee should be sent. Generally, this is where the payroll for this employee is processed. Once this data has been entered into our system, it will be attached to future submittals. This address may be modified or additional addresses may be added, but be sure to report each New Hire under their associated address.

		Employer Street 1:
		The first line of the street address of the Payroll/Child Support Withholding Address.

		Employer Street 2:
		The second line of the street address of the Payroll/Child Support Withholding Address (if needed).

		Employer Zip:
		The zip code of the Payroll/Child Support Withholding Address. This must be a valid zip code.

Errors/How to Correct Errors

	All data is edited prior to submittal to ensure completeness and accuracy. If any required data is missing or fails a validity check, you will need to correct the data before the new hire can be submitted.
